What is Supported Living?

Since ElderHealth Northwest's founding over 25 years ago, participants of our programs have encouraged us to offer a residential living option. After much careful planning and the search for suitable locations, we are pleased to offer our innovative "Supported Living" in our gracious historical home on Seattle's Capitol Hill and in our new home in Seattle's vibrant Columbia City district.
ElderHealth's Supported Living is for people with memory loss or other health conditions who don't want to leave their homes for an institutional setting and who need more personal attention than can be found in a typical large-scale facility.

Most residents have memory loss and many have other health conditions, such as diabetes, Parkinson's disease, heart conditions, or stroke. We provide assistance throughout the day: at mealtime, in the bathroom, with taking medications, with bathing, dressing, and walking/getting around.

ElderHealth can accommodate individuals at various levels of care. Some residents need only occasional cueing; many require frequent monitoring and hands-on assistance.

Secure and Safe

Paramount in our design is the needs of persons with memory loss. Wandering safeguards in the homes and gardens ensure the safety and security of our residents. Our residences are staffed 24 hours a day, every day of the year. Members of our staff team are awake and available throughout the day and night.

Staff

Both homes have a staff team that includes the Manager, Nurse, Resident Assistants, and Housekeeper.

Meals

Meals are prepared on-site, using seasonal menus guided by residents' favorite recipes.

Services

Every resident has an individualized care plan. We emphasize one-on-one interaction, and tailor our activities and programs to the needs of the individual. Our nurse will perform a range of treatments within our licensing parameters. We can assist families in arranging additional services as needed. Our services include:

  • Three nutritious meals, snacks and beverages daily
  • Housekeeping and linen service
  • Health monitoring
  • Fun and therapeutic activities
  • Medication assistance
  • Incontinence care, including routine colostomy and ostomy care
  • Assistance with bathing, dressing and grooming
  • Companionship and reassurance
  • Ambulation assistance
  • Meal-time assistance
  • Get-up and tuck-in help

Gaffney House

Buchanan Place

Errand running and staff escorts (to and from appointments) are available to Gaffney House residents on a fee-for-service basis. Residents who need physical/occupational therapy or more intensive nursing services may enroll in our neighboring adult day health center.

This beautiful home was built in 1903 and is listed in the National Register of Historic Places. Located on Capitol Hill at 17th and Madison, the home is especially convenient for adult children and relatives who live or work in Seattle. We can accommodate up to 16 residents; two of the 14 rooms have been set aside for couples.

Set in a quiet residential neighborhood near the center of Columbia City, our new 12-person home is directly adjacent to ElderHealth Northwest's South Branch adult day health center. Nearby Columbia Park offers open grass and magnificent maple trees. The neighborhood also hosts arts and cultural events, including the seasonal Columbia City Farmers Market and annual Rainier Valley Heritage Parade.

Eligibility and Inquiries

For Buchanan Place, residents must be on COPES (assistance for low-income persons with disabilities) and have case manager approval for the move-in. For Gaffney House, residents must have financial means to private-pay. Call (206) 528-5315 for details.
