ü      Medicare ID #

ü      Full Address

ü      Preferred Pharmacy

ü      Pharmacy Address

ü      List of Drugs, Dosage and Frequency

ü      Any recent letters you have received

      from Social Security or Medicare

ü      Information on your current  

     prescription drug coverage

 

 

Representatives from Senior Services, Center for Medicare Services and the Social Security Administration will also be present.
